Piroxicam before embryo transfer improves success rates: study

NEW YORK (Reuters Health) - For women undergoing in vitro fertilization and embryo transfer (IVF-ET), priming the uterus with a single oral dose of piroxicam just before embryo transfer markedly increases the implantation and pregnancy rate, fertility specialists from Korea report in the October issue of Fertility and Sterility.
Nonsteroidal anti-inflammatory drugs (NSAIDs), such as piroxicam, inhibit production of prostaglandin and may therefore reduce uterine contractility, Dr. Hwa Sook Moon and associates from the Center for Reproductive Medicine and Fertility at the Good Moon-Hwa Hospital in Busan explain in the paper.
They studied the effects of 10 mg oral piroxicam given 1 to 2 hours before ET in 94 fresh ET cycles and 39 frozen-thawed ET cycles. In a like number of fresh and frozen-thawed ET cycles, patients received oral placebo before ET.
In fresh cycles, piroxicam significantly increased the implantation rate (18.7%) and the clinical pregnancy rate (46.8%) compared to placebo (8.6% and 27.6%, respectively).
Likewise, in frozen-thawed ET cycles, significantly higher implantation and clinical pregnancy rates were achieved with piroxicam (9.4% and 25.6%) than with placebo (2.3% and 7.7%).
The benefits of piroxicam were most pronounced in women younger than age 40 with tubal, male infertility, or endometriosis factors.
"These results suggest that administration of piroxicam just before ET is very effective in the priming of the uterus to be suitable for embryo implantation," the authors conclude.
Further study is needed to determine whether piroxicam calms the uterus by reducing uterine contractions or if it increases uterine blood flow, they add.
